Under the initial assessment heading, record the patient's mental state/well being and the level of distress they're experiencing.
For recent side effects, briefly record the most significant toxicities but all side effects should be recorded with grading on the Visit Record


Record whether patient consent has been obtained, whether this is a new patient and if you've discussed the method of transport home with the patient
