4.0 Risk Assessment - Nursing
The Risk Assessment (Nursing) has four sections:
Pressure Risk assessment VTE risk Infection status Delirium and cognitive impairment assessment (4AT score)
The pressure risk assessment has 7 assessments with 4 options in each. They create a total Norton Pressure Risk Score and a Pressure risk category for the patient.
VTE risk
Options for adding more detail are provided depending on the answers to the questions given.
Infection status
Record the patient's risk of infection with CJD and whether they're carrying or infected with a multi-resistant organism. The infection entered will display on the Record Summary
Delirium and cognitive impairment assessment
For the question 'Include delirium / cognitive impairment assessment, if 'Yes' is selected, more fields will display with questions to complete.
A 4AT Score will be calculated depending on answers and a delirium score category will be assigned.
