8.0 ADDIKD - Anti-cancer drug dosing in kidney dysfunction - Carboplatin dosing

Carboplatin dosing:

There are three choices of units for Carboplatin dosing that all use the Calvert equation, but each use a different method to calculate kidney function:

When new protocols containing Carboplatin are created, the default units will be BSA-adjusted eGFR, but this can easily be changed by editing the medication to the preferred units and carrying the choice forward to all future cycles.

The Cancer Institute states that ‘Directly measured glomerular filtration rate (mGFR [mL/min]) is the preferred kidney function value in the Calvert formula, especially where estimated kidney function may be unreliable for accurate therapeutic dosing.’ As this requires a 24-hour urine collection, it’s assumed that many patients will not have an mGFR result, hence the decision to make BSA-adjusted eGFR the default method.
