Changes to Cigarettes and Alcohol Questions (BE-158)

Under the parent question 'Do you drink alcohol? *':
- Changed 'How many standard drinks per day? ' to 'How many drinks per day (on the days that you do drink)'
- Changed 'How often? ' to 'How many days a week do you drink?'

Under the parent question ' Smoking Status (when selecting either 'Current Smoker' or 'Past Smoker'):
- Change the question 'Amount per day *' to 'Daily Amount of Cigarettes *'

New label for 'Add Medication' Button and New Tooltip next to the 'Delete' Medication Button (BE-159)

Changed label of 'Add Medication' button to 'Click to add a medication'

Added tooltip next to the red 'X' delete button with 'This button will delete this row of medication details'

Change 'Other Entitlements' heading to 'Concession Cards' and add a sample image (BE-160)

Changed the heading of 'Other Entitlements' to 'Concession Cards'

Added an instructional image to 'Other Card Type' to assist patients to enter in their entitlement details

Add label to 'Date of Birth' field (BE-161)

Added instructional text: 'We suggest typing in your date of birth - please use format dd/mm/yyyy'

Add fields to Copy Forward function (BE-162)

1. Insurance Details page (all fields)
2. GP's First Name - Located on 'This Hospital Visit' page
3. GP's Surname - Located on 'This Hospital Visit' page
4. GP's Postcode - Located on 'This Hospital Visit' page
5. GP's Suburb - Located on 'This Hospital Visit' page

Please Note: These have been moved to Personal Details. page 2 to allow them to be part of the copy forward function as nothing can be copied forward before an Admission ID based off Admission date and admission type. (what does this mean? Use customer-friendly wording)

Allow future dates up to Admission date for Pathology Tests question (BE-163)

Allow the user to select a date in the 'When?' field (see screenshot) that:
1. Is in the future
2. BUT IS NOT a date in the future that is after the 'Admission Date' selected on page 1 of the Admission Form


e.g. If a patient has selected an admission date of the 1st of April 2021, but is completing their form on the 1st of March 2021 they should be able to select dates in this field up to and including the 1st of April 2021 but not the 2nd of April onwards.


The rationale of this change is that users who are completing their admission details ahead of time but have not yet had a relevant test or tests can enter the details of those tests accurately.

Add wording to Admission Form Submit page and History Form Submit page (BE-164)

Add warning for users that their response is final and they can navigate back through the pages to make any changes (See screenshots).

On the Admission Form Submission Page add new text: 'Please note that you can use the dropdown menu under 'Admission Form' above to go back and edit any of your previous answers.'

On the History Form Submission Page add new text: 'Please note that you can use the dropdown menus under 'Admission Form' and 'History Form' above to go back and edit any of your previous answers.'

Change 'Copy' button colour and add text to make it more prominent (BE-165)

Change the background colour of the 'Copy' button on the Home page as it appears in the list of admissions to Burnside yellow and change the text of the word 'Copy' to white

Add the following text above the 'Start New Admission' button: 'If you have a previous admission listed below, use the COPY(Display image) button in the table to pre-load your personal data'

Add a warning label for Mobile Phone Number' field when creating an account (BE-166)

When creating their account patients sometimes put in the wrong mobile number the task is to add warning text to the phone number field so that it is clear for patients.
Add the following warning text under the 'Mobile Phone Number' field on the 'Register' page: 'Please ensure you enter the correct mobile phone number as this will be used to validate your account registration and your Hospital form submissions' (See screenshot)



Add warning text to Insurance Details question (BE-168)

Add new warning text 'Please advise your insurance details for this admission ' on the 'This Hospital Visit' page add the following text: "Please note you will be asked for further details on the Insurance page"*

Change text under Admitting Dr Lookup (BE-169)

Change the wording under Admitting Dr field which is currently labelled: 'Search Dr surname and select from list' to 'Please type the first few letters of the Doctor's surname and select from the drop down list displayed'

Add more text to warning label when attempting to register with an existing Email Address (BE-170)

Add the following wording to the error message in addition to the existing wording: 'Each patient requires a unique email address for security reasons.'

Wording changes to 'Other Card' field names (BE-172)

Change the wording of 'Other Card Type' to 'Concession Card Type'

Change the wording of 'Other Card Number' to 'Concession Card Number'

Change the wording of 'Other Card Expiry' to 'Concession Card Expiry'

New DVA Card Prefix: NV (BE-156)

DVA GOLD cards are now valid with the prefix of 'NV', followed by 6-digits

New DVA Warcode of V (BE-148)
