Optional 'Frequently Asked Questions' link at footer of Patient Portal

This redirects patients to an external webpage or document that the health provider can curate (usually a page on their website), which is intended to reduce patient support calls to the health provider.


CHANGES TO QUESTIONS


Admission Form


Admission Details page:

Update to Existing Question - 'How did you find out about this hospital? (ADMISSION_DETAILS_HEARD_ABOUT_HOSPITAL)

When active, the user will be asked to Specify how they heard about hospital, if they select 'Other' from the dropdown list
New, Optional Format for Admitting Dr Entry - Show/Hide Suburb & Postcode (ADMISSION_DETAILS_INCLUDE_ADMITTING_DR_SUBURB_POSTCODE)

When active, this displays the Admitting Dr suburb & postcode fields. Conversely, it hides these two fields when it is inactive.


Bug Fix - Previous Hospital Visits where the 'Previous' Visit hasn't happened in the past

For patients who have two future admissions, and are trying to enter the first future admission date as a previous hospital admission on the second future admission, there was a bug preventing them from doing so.

This has been fixed by allowing any future date to be entered on a previous hospital admission.

Personal Details page:


Patient Gender question re-worded: 'Patient Gender' question has now been reworded to 'Patient sex'.



New Question Available - Identifying Pronoun (PERSONAL_DETAILS_INCLUDE_IDENTIFYING_PRONOUN)

When active, this will include an optional question asking the patient for their preferred identifying pronoun.


Update to Existing Question - Preferred / identifying Name (PERSONAL_DETAILS_INCLUDE_PREFERRED_NAME)

Where the question is active, the existing 'Preferred Name' field has been relabelled 'Preferred / Identifying Name'.


New Question Available - Additional Genders (PERSONAL_DETAILS_INCLUDE_GENDER_ADDITIONAL)

When active, this question will capture additional gender options, if applicable to the patient.



New Consents Available - SMS notifications and patient satisfaction survey consents

When active, one or both of the following configurable consents can display on the Personal Details page:

1. Consent for patient to receive SMS notifications about their admission
2. Consent for patient to receive a patient satisfaction survey


Personal Details (Additional) page:

New, Optional Format for Country of Residency (DEMOGRAPHICS_ADDITIONAL_RESIDENCY_ALTERNATE_FORMAT )

Instead of a drop-down list for the patient's country of residency, the question is re-formatted to ask if the patient is an Australian Resident or not.


GP Details page:

New Question Available - My Health Record (GP_DETAILS_INCLUDE_MY_HEALTH_RECORD)

When active, patients are asked if they want to consent to the uploading of their admission information to their My Health Record account.




New Question Available - Notify GP (GP_DETAILS_INCLUDE_NOTIFY_GP)

When active, patients will be asked if they want their GP notified of their admission and discharge.

New Questions Available - Referring Doctor (if not GP) (GP_DETAILS_INCLUDE_OTHER_REFERRER)

When active, patients will be asked if their entered GP referred them to their admitting doctor. If not, they can provide those additional referrer details.





Updated Instructions for Entering GPs

Text has been added above GP Surname in the GP Details section to improve compliance of patients entering in their GP.

Persons to Contact page:

'Lives Overseas' tick-box now displayed for NOK contacts: This means that patient can now enter an overseas address for their NOK contacts



New Custom Persons to Contact page:

A new custom page option has been developed to cater to patients who may not have a NOK.





Insurance Details page:

New Question Available - Insurance 'Level of cover' (INSURANCE_INCLUDE_LEVEL_OF_COVER)

When active, Private Health Funded patients are asked about their Health Fund level of cover




New Questions Available - Third Party Accident Details

(INSURANCE_INCLUDE_ACCIDENT_LOCATION, INSURANCE_INCLUDE_CAUSE_OF_INJURY, INSURANCE_INCLUDE_CLAIM_APPROVED)

When active, these optional questions inquire about the patient's third-party accident details, including accident location, cause of injury and if their insurance claim is approved.


Legal page:

New Question Available - Medical Power of Attorney (LEGAL_INCLUDE_MEDICAL_POWER_OF_ATTORNEY)

When active, a new mandatory question captures the patient's 'Medical Power of Attorney'.





Optional Rewording of Existing Questions - 'Enduring Guardian' and 'Power of Attorney' questions (LEGAL_ALTERNATE_LABEL_FOR_PERSONS)

When active, this rewords these two legal person questions, as per the screenshot below.

New Question Available - Registered Organ Donor (LEGAL_INCLUDE_ORGAN_DONOR)

When active, this displays a mandatory question "Are you registered with the Australian Organ Donor Register?*





New, Optional Format Available - Show/Hide Financial Implications Question on HC21 (LEGAL_INCLUDE_FINANCIAL_IMPLICATIONS_QUESTION_HC21)

When active, this displays the Financial Implications question on the Legal Page, which maps to the custom 1-page HC21 printout.

Conversely, when inactive, it hides this question and no response is mapped to the printout.


New Custom Page Available - 'Legal for Minors' page for Paediatric Patients

For Paediatric patients, this replaces the standard 'Adult' Legal page. The only difference with the Adult page is that instead of the Legal Information section which asks about Advance Care Directives, Power of Attorney etc. , the Legal Minor page asks about custody / guardianship orders or visitation restrictions that may affect the paediatric patient.

Allergies and Medications page:

New Question Available - Food Allergies (ALLERGIES_MEDS_INCLUDE_ALLERGY_TO_MEDICATION_FOOD)


When active, a new mandatory question "Do you have any food allergies?* is displayed. This is designed to remind patients of any food allergies they may have, in response to health provider feedback that this type of allergy is often omitted by patients, until they are asked specifically about them.



New Question Available - Usual Method of Medication Administration (ALLERGIES_MEDS_INCLUDE_MEDICATION_ADMINISTRATION)

When active, the patient is asked about their usual method of medication administration (syrup (cup/syringe), tablets, suppositories, other).

New Question Available - Medication Assistance (ALLERGIES_MEDS_INCLUDE_MEDICATION_ASSISTANCE)

When active, the patient is asked if they require medication assistance.


Update to Existing Question - Any Other Medications

The question that triggers the medications grid "Are you taking any other medications?" has been re-labelled to "Are you taking any other prescription medications or non-prescription medications or complimentary medicines including vitamins / mineral / herbal remedies?"

This change was made in response to health provider feedback that patients often forget about over-the-counter or complimentary medicines, which are important for the clinical preadmission team to know. Hence this updated wording serves as an added reminder for the patient.

Optional Reformatting of Existing Question - Steroid Medications

When inactive (the default is Active), the Steroid Medications question can be re-formatted to hide the Admitting Dr informed and Cessation Instructions subquestions.

New, Optional, Custom Page - Discharge Planning for Paediatric Patients

This page has a tailored question set for Paediatric patients focused on who will be looking after the patient during their admission. The full question is shown below.
