Reduce confusion for paediatric SAH Day Surgery Hornsby patients when they are selecting their admission type

SAH Day Surgery Hornsby (SDSH) patients were confused by the label of the only admission type they can see - ‘Adult’ - when SDSH patients admit Paediatrics too. SDSH wants both Adult and Paediatric patients to use the same admission questionnaire.

SAH (main campus) have a different admission form flow for their Paediatric patients compared to their Adult patients.

There is also a static instruction for SDSH and SAH (main campus) on the page ‘Please select Admission Type of Paediatric if the patient is 16 years or under.’ which further confuses the patient as they can’t follow the instruction.

The list of admission types is common to both SDSH and SAH (main campus).

To resolve this confusion, the existing 'Adult' admission type was relabelled to 'Day only (same day) which will continue to be offered to both SAH and SDSH patients.



Improve user-experience for pensioner card entry
To reduce patient confusion with the expected format for Pensioner Card entry, an instructional image has been added.


Improve user-experience for Medicare card entry

To reduce patient confusion with the expected format for Medicare Card entry, an instructional image has been added.





Update medication entry instructions for patients

At the request of SAH clinical staff, The instructions above the Medication Grid on the Allergies & Medications page has been re-worded to the following (red-text indicates the change)

Please include tablets, capsules, puffers, nebulisers, patches, insulin, eye drops. Consult your GP or surgeon if you are unsure of any details about your medications or which medications should be ceased prior to your surgery. Bring into hospital ALL current medications you are taking in their original packaging or in a sealed and labelled dose administration pack (e.g. Webster® or roll sachet). Also bring in prescriptions for current medications and PBS entitlement cards.


Expand wording of Short term memory loss / dementia to include delirium / development delay (PastMedicalHistory page)

This was a request by SAH clinical staff as:

1. Asking about delirium is a recent accreditation requirement for the SAH, and;
2. Development delay is on their paper admission form but not on eAdmissions




Support SDSH-specific contact details in the admission form submitted email confirmation

The Admission Form Submitted message template does not support parameterised contact details, so the email for SAH Day Surgery Hornsby (SDSH) patients incorrectly includes the ‘static’ contact details for the SAH main campus.

By parameterising the contact details in this email template, the email confirmation for SDSH now shows specific and correct contact details for those patients.
