PATIENT PORTAL
Two factor Authentication on the epi-me Patient Portal (EM-5657 and EM-5658)
Episoft have developed two-factor authentication for the epi-me Patient Portal, in order to improve the security of a patient's account and their admission and clinical forms. This can be configured on each epi-me patient portal at the customer's request. When registering, users can select their preference for two-factor authentication. This sends a code to the user's email or mobile phone upon login, based on their set preference. This ensures the user's account is more secure by verifying the user's details via both a password and the authentication code sent to their email or mobile.
Users can choose to have a 6 digit code sent to either their email or mobile phone and after entering their email and password they are prompted to enter it. Each hospital customer can decide the number of days that users will be remembered on the browser.
Additionally, existing users are prompted to select their preference for two-factor authentication when they next login and a user's preference for the code to be sent to their mobile phone or their email can also be changed upon login using the 'My Details' tab in the top left of the patient portal home screen.
Admission Details Page - New Wording on the Previous Visits question (EM-5911)
Episoft have added a new Form configuration which amends the previous visit question on the Admission Details page. By setting ADMISSION_DETAILS_USE_HOSPITAL_ALTERNATE_WORDING_AND_SUBQUESTION to true, hospital customers who use this question have the wording amended to 'Have you previously been an inpatient or outpatient at XXX Hospital?'
Admission Details Page - MBS Item Numbers (EM-5992)
Episoft have added a new Form configuration, labelled ADMISSION_DETAILS_INCLUDE_MBS_ITEMS, which when set to True displays the MBS item number lookup field. Customers who do not want this field are able to set the form configuration to False.
Admission Details Page - Hide Previous Visit section (EM-6078)
Episoft have added a new Form configuration, labelled ADMISSION_DETAILS_PREVIOUS_VISIT_SECTION_INCLUDE_28_DAYS which is True by default but when False hides the entire previous hospital visits section.
Personal Details page - Add new 'Sydney Contact Number if not from Sydney' (EM-5921)
At the request of a hospital customer Episoft have created a new form configuration labelled PERSONAL_DETAILS_ADD_SYDNEY_CONTACT_NUMBER) which when set to True adds an additional contact number field when TRUE labelled: 'Sydney contact number(s) if not from Sydney'
Personal Details(continued) page - Reconfigure the Employment Status drop down as a free text field labelled Occupation (EM-5922)
At the request of a hospital customer Episoft have created a new form configuration labelled labelled DEMOGRAPHICS_ADDITIONAL_RELABEL_RECONFIGURE_OCCUPATION which will remove the current ‘Employment Status’ DDL and replace it with a free text field labelled ‘Occupation’. (NOTE: This field is nested under: DEMOGRAPHICS_ADDITIONAL_SHOW_OCCUPATION)
Insurance Details page - New hint text on the Medicare Expiry field (EM-6458)
There is a new Form configuration to add hint text next to the meciare expiry fields to display the format of the expiry field to patients on the Insurance page. These will display when the following configuration is set to True: INSURANCE_ALTERNATE_MEDICARE_EXPIRY_LABEL
Insurance Details page - New Alternate 'Other Cards' section (EM-5919)
A new Form Configuration has been added which reconfigures the Other Cards section and displays each by default, INSURANCE_OTHER_CARD_ALTERNATE_DISPLAY_THREE. Setting this to True splits out the various 'Other Card' types into distinct questions as follows:
- Do you have a pensioner card? (Y/N) with subquestions: Valid pension card number and pensioner card expiry (it also displays a sample card as an image)
- Do you have a health care card? (Y/N) with subquestions: Valid health care card number and health care card expiry
- Do you have a Commonwealth seniors card? (Y/N) with subquestions: Commonwealth seniors card number and Commonwealth seniors card expiry
Insurance Details page - New field of Insurer's fax number added to WC, Third Party and Public Liability (EM-5919)
A new Form Configuration has been added at the request of one of our customers to include an 'Insurer's fax number' field for users who select Workers Compensation, Third Party and Public Libaility. This can be configured by setting INSURANCE_INCLUDE_INSURER_FAX_NUMBER to True.
Insurance Details page - PRFA address fields (EM-6011)
A new Form Configuration has been added at the request of one of our customers labelled: INSURANCE_INCLUDE_PAYMENT_ACCOUNT_POSTAL_ADDRESS
When set to True (it is False by default) this will display a mandatory question: ‘Postal address same as residential? *' (Y/N) located below the email address field (see screenshot below) - Note: This is changed from V2 as the address fields do not apppear unless ‘Relationship to patient *’ has been selected as ‘Other’ in V3
If the ‘No’ radio button is selected it will display the following fields:
- Address * - addressify lookup
- Can't find address? (tickbox as per previous address fields)
- Building name and/or Unit No.
- Street No. * (SPEC INCORRECT SHOULD BE OPTIONAL ON POSTAL ADDRESS)
- Street Name *
- Suburb *
- Postcode *
- Live overseas?
- State
Legal Minor page - New Form Configurations to hide Custody Orders and include questions on Visitor and Phone Restrictions (EM-5950)
We have added three new Form Configurations to the Legal minor page for Paediatric admissions. The first (LEGAL_MINOR_INCLUDE_CUSTODY_ORDERS) is defaulted to True for epi-me customers but for customers who want to hide the question 'Are there any custody orders regarding the patient?' can set this to False. The second Form Configuration (LEGAL_MINOR_INCLUDE_SPECIFY_FOR_VISITOR_RESTRICTIONS) adds the following question when set to True: Are there any visitation restrictions we should be made aware of? *. The third Form Configuration (LEGAL_MINOR_INCLUDE_PHONE_RESTRICTIONS) adds the following question when set to True: Do you wish to have any restrictions on telephone? *
Admission Form Submission Page - New Form Configuration to make Patient rights and responsibilities consent mandatory (EM-6150)
Episoft have added new Form Configuration which allows our epi-me customers to make the patient's rights and responsbilities consent mandatory. This is governed by ADMISSION_SUBMIT_RIGHTSRESPONSIBILITIES_CHECKBOX_MANDATORY which when set to True makes the consent mandatory.
Health History - Relabel of Skin Integrity question wording to use correct clinical terminology of 'Injuries' (EM-5598 and EM-6465, Helpdesk #23241)
At the recommendation of one of our epi-me customers Episoft have universally relabelled the skin integrity question for those hospital customers who have it configured to 'Do you have or have you ever had skin injuries or pressure injuries? *'. This was done to accord with established clinical wording.
Health History - New Form Configuration to relabel Cancer question (EM-5785)
Episoft have added new Form Configuration which allows our epi-me customers to relabel the primary Cancer question to 'Cancer / lymphoma / leukaemia *'. This is False by default for all customers but can be configured by setting the following Form Configuration to True: HISTORY_SIGNIFICANT_CANCER_ALTERNATE_LABEL
This will then display the question to Patients as follows:
In addition to this a new form configuration has been added to make the 'Date of Inital diagnosis' on the Cancer section mandatory. This field only appears when the existing Form Config labelled: HISTORY_SIGNIFICANT_INCLUDE_CANCER_DATE_LAST_TX is set to True.
The new configuration is labelled HISTORY_SIGNIFICANT_CANCER_DATE_LAST_TX_MAKE_MANDATORY and is nested under the other configuration that sets the date as mandatory.
Health History - New Dynamic Text Item above Surgeries grid (EM-5785)
Episoft have added a new Dynamic Text item (number 92) above the Surgeries grid on the 'Other Significant History' page. Customers are able to enter their desired wording to display to patients making it clearer that they need to select the 'Add Previous Surgery' button. This wording is configurable using the Dynamic Text page of the Admin Portal.
Health History (Paediatric) - New Form Configurations on the Paediatric Infection Screening page (EM-5954)
A new Form configuration (HISTORY_INFECTION_INCLUDE_HOSPITAL_OS_LAST_12_MO) has been added to hide the 'Has the child been overseas in the last 12 months?' question. When set to True this question will display.
A new Form configuration has been created to relabel the respiratory question on the Paediatric infection screening page to 'Has your child recently had a cough, cold or contact with an infectious disease.' This is labelled HISTORY_INFECTION_COUGH_FEVER_RELABEL_INCLUDE_INFECTIOUS_DISEASE and is defaulted to False for all epi-me customers.
A new Form configuration has been created (HISTORY_INFECTION__CJD_RELABEL_FAMILY_HISTORY) this will display the alternate wording of 'Does your child, or any relatives, have Creutzfeldt-Jakob Disease (CJD)? *' when set to true, but is defaulted to False for all epi-me customers.
A new Form configuration has been created (HISTORY_INFECTION_EXTENDED_CJD_SECTION) it is defaulted to False. This displays the following questions: Does your child have an unexplained progressive neurological illness of less than 12 months duration? * (with a please specify field) and Has your child been involved in a ‘look-back’ for Creutzfeldt Jakob Disease (CJD) or have you been shown a ‘medical in confidence’ letter regarding their risk for Creutzfeldt Jakob Disease (CJD)? *
A new Form configuration has been created (HISTORY_INFECTION_INCLUDE_SPECIFY_FOR_IMMUNISATIONS) which when set to False will remove the 'Please specify' textbox that appears when the user hits no to 'Is your child fully immunised? *'. This is true by default.
Health History - New form Configuration to Hide Multi-Resistant Organism section on the Adult Infection Screening page (EM-5788)
At the request of an epi-me customer we have added a Form Configuration to hide the MRO section and all sub questions. This is by default False for all customers but can be set to True and is governed by the Form Configuration: HISTORY_INFECTION_INCLUDE_MRO
Health History - New form Configuration to Combine the Creutzfeldt Jakob Disease and Neurosurgery questions on the Infection Screening page (EM-5788)
At the request of an epi-me customer we have added two new Form Configurations to combine the previous neurosurgery and previous dura mater graft questions into a single combined question and hide the year field in which a patient was given human pituitary growth hormone. The first Form configuration is labelled: HISTORY_INFECTION_COMBINE_CJD_HORMONE_AND_NEUROSURGERY_QUESTIONS
and the following Form Configuration hides the 'Year' field for HISTORY_INFECTION_INCLUDE_CJD_GROWTH_HORMONE_YEAR
Health History - New form Configuration to amend the Recent Cold / Flu / Sore Throat question on the Infection Screening page (EM-5788)
At the request of an epi-me customer we have added a Form Configuration to amend the Recent cold / flu / sore throat question to more cutdown wording which reads: 'Recent cold *'. This question is amended by setting the following Form Configuration to True, and is False by Default for customers: HISTORY_INFECTION_FLU_ALTERNATE_LABEL
Health History - New form Configuration to hide the Vomiting and Diarrhoea questions on the Infection Screening page (EM-5788)
At the request of an epi-me customer we have added a new Form Configuration to hide the vomiting and diarrhoea question on the infection screening page, this will be True by default for customers and is labelled: HISTORY_INFECTION_INCLUDE_VOMITING_AND_DIARRHOEA
Health History - New form Configuration to add Herpes and Rubella questions to the Infection Screening page (EM-5899)
At the request of an epi-me customer we have added two new Form Configurations to includes questions on Herpes and Rubella/German measles to the infection screening page. The first Form Configuration is labelled HISTORY_INFECTION_INCLUDE_HERPES_SECTION_TYPES1AND2 and when set to True displays the following questions with associated Y/N radio buttons: 'Herpes Type 1*' and 'Herpes Type 2*'. This new question will trigger the Infection Alert.
The second Form Configuration is labelled HISTORY_INFECTION_INCLUDE_RUBELLA_SECTION_TWO_QUESTIONS_HAD_DISEASE_AND_BEEN_VACCINATED and when set to True displays two questions on the infection screening page: Have you had rubella (German measles) *' with radio buttons for 'Immune', 'Not Immune' and 'Not sure' and in addition the question: 'Have you been vaccinated against rubella (German measles)? *' with radio buttons for 'No', 'Yes' and 'Not sure'. This new question will trigger the Infection Alert.This configuration is False by default.
Health History - New form Configuration for Maternity Patients on the Lifestyle and Diet page (EM-6220)
On the Lifestyle and Diet page for Maternity admissions, we have added a new Form Configuration (HISTORY_LIFESTYLE_PREGNANT_INCLUDE_NUMBER_OF_WEEKS_ with a default setting of False. When set to True it will ask patients who select 'Yes' to are you or could you be pregnant a seaparate question albelled 'number of weeks'.
Health History - New form Configurations on the Lifestyle and Diet page (EM-5793)
At the request of an epi-me customer we have added a number of new Form Configurations to the Lifestyle and Diet page, these make changes to the existing questions and are available to epi-me customers after their next release.
The first is a change to question to the question on recreational drugs and relabels the question to 'Past history of drug dependency *' as well as relabelling the sub question. This is governed by the new form configuration: HISTORY_LIFESTYLE_USE_RECREATIONAL_DRUGS_ALTERNATE_LABEL which is False by default.
The second change is to the existing exercise frequency question which all customers currently have but can now be hidden as an option by using the Form Configuration: HISTORY_LIFESTYLE_INCLUDE_EXERCISE_FREQUENCY. This True by default.
The following optional Form Configurations make changes to the Aids, Impairments and Mobility section. The first adds two checkboxes to the 'Visual Aids / Impairments' question, 'Left' and 'RIght' so users can specify which eye has an issue or impairment. This is governed by HISTORY_LIFESTYLE_VISUAL_AIDS_INCLUDE_LEFT_RIGHT_CHECKBOXES which is False by default but available to epi-me customers optionally.
The second optional change allows for the 'Glaucoma' and 'Eye Lens' checkboxes under the same Visual Aids / Impairments question be hidden by setting the Form Configuration HISTORY_LIFESTYLE_VISUAL_AIDS_INCLUDE_GLAUCOMA_AND_EYE_LENS_CHECKBOXES" to False. This is true by default for customers.
The third optional change is the optional removal of the primary mobility question. This is governed by the new form configuration HISTORY_LIFESTYLE_INCLUDE_IMPAIRMENT_MOBILITY" and is by default True for epi-me customers.
The fourth optional change is a change to the 'Walking Aids' question which when set to True amends the question from a series of optional checkboxes e.g Walking Frame, Stick etc. to a (Yes/No) radio button with a mandatory 'Please specify' field. This is governed by setting the following Form Configuration to True: HISTORY_LIFESTYLE_AIDS_MOBILITY_WALKING_ALTERNATE_LAYOUT.
On the Lifestyle and Diet page there is a new optional Form Configuration which hides the Skin Conditions section and all the checkboxes within it e.g Eczema, Psoriasis etc. This is governed by the following form configuration and is True by default for epi-me customers. HISTORY_LIFESTYLE_INCLUDE_SKIN_CONDITIONS.
On the Lifestyle and Diet page there is a new optional Form Configuration which amends the dental section and asks about Dentures only, with (Y/N) radio buttons for 'Dentures - upper' and Dentures - lower *' and checkboxes for 'Full' and 'partial' nested under each. This can be enabled by setting the following Form Configuration to True: HISTORY_LIFESTYLE_DENTAL_SECTION_INCLUDE_DENTURES_ONLY_WITH_DENTURES_DETAIL
Lifestyle & Diet (Adult & Paediatric) - new hint text to add food allergies to the Allergies Grid rather than special diets (EM-5793)
This is only applicable to any customers with the standard Paediatric Lifestyle & Diet page, or the Adult Lifestyle & Diet page with HISTORY_LIFESTYLE_INCLUDE_SPECIAL_DIETS set to TRUE (i.e Special Diets being collected for Adults).
We have introduced helpful hint text to instruct Patient Portal users to enter any special dietary needs relating to food allergies on the Allergies Grid rather than as a special diet.
This is not subject to a configuration and has been added as a global enhancement.
Health History - New form Configurations on the Discharge Planning page (EM-5794)
There have been a number of new Form Configurations added to the Discharge Planning page that are available as optional changes to epi-me customers. They include a new Form Configuration that allows a hospital customer to hide the question: 'Do you have any caring responsibility for others?'. This is governed by the following Form Configuration which is set to True by default HISTORY_DISCHARGE_CARE_FOR_OTHERS_INCLUDE_SPECIFY
The Form Configuration (HISTORY_DISCHARGE_ADLS_RELABEL_COMMUNITY_SERVICES) when set to True relabels the 'Do you require assistance with daily activities?' question to: 'Do you receive community services? *'. and in addition hides the checkboxes in that section that are not community services (see below screenshot). This will be False by default for most epi-me customers.
The new Form Configuration (HISTORY_DISCHARGE_CONCERNS_HOME_RELABEL_PROBLEMS_CARING) when set to True relabels the caring responsibilities question as 'Do you have problems caring for yourself at home? *'. This will be False by default for most epi-me customers. In addition a new Form Configuration has been added that allows the associated 'Please specify' question to be removed when the following form configuration is set to False: HISTORY_DISCHARGE_CONCERNS_HOME_INCLUDE_SPECIFY.
There is a new Form Configuration which allows epi-me users to detail their relationship with the person taking them home by selecting from a drop down list (this displays for Day Stay patients only). This can be configured by setting the following Form Configuration to True: HISTORY_DISCHARGE_INCLUDE_RELATIONSHIP_OF_PERSON_TAKING_HOME. In addition epi-me hospital customers can collect a mobile phone number of the person responsible for collection by setting the following Form Configuration to True: HISTORY_DISCHARGE_INCLUDE_MOBILE_PHONE_DAY_ONLY.
At the request of one of our customers Episoft has also created a form configuration to hide the final 'any other medical conditions' question on the discharge planning page, this can be removed by setting the following to False: HISTORY_DISCHARGE_INCLUDE_MEDICAL_CONDITIONS. This is True by default for epi-me customers.
Health History - New form Configurations on the Obstetric History page (EM-5900 and EM-6236)
A new set of Form configurations have been added to the Obstetric History page and can be optionally configured by epi-me customers. The frist is a form configureation that sets the patient's partners name to a single field (as opposed to First and Surname as distinct fields) This can be configured by setting the following Form Configuration to True: HISTORY_OBSTETRIC_INCLUDE_PARTNER_NAME_ONE_FIELD_ONLY.
A new form configuration which displays a free text field labelled 'Other Relevant Past medical history' can be added to the Obstetric History page by setting the following configuration to True: HISTORY_OBSTETRIC_INCLUDE_RELEVANT_PAST_HISTORY_FREE_TEXT
A new form configuration to include the following question has been added: 'Are you having a planned caesarean section? *'. This can be added by setting HISTORY_OBSTETRIC_INCLUDE_CURRENT_PREGNANCY_IS_A_CAESAREAN to True.
At the request of one of our customers we have added a new form configuration to allow for the question 'How many weeks pregnant were you at the time of your first appointment' to be hidde. This can be implemented by setting the following Form Configuration to False HISTORY_OBSTETRIC_INCLUDE_FIRST_VISIT_TO_DR_WEEKS_PREGNANT. This configuration is True by default for epi-me customers.
Episoft have developed a new Form Configuration for the multiple pregnancy question to allow users to enter the number of expected babies, with the wording 'If you are expecting more than one baby, how many?'. This can be configured by setting the following Form Configuration to True: HISTORY_OBSTETRIC_ALTERNATE_QUESTION_FOR_MULTIPLE_BIRTHS_AS_NUMERIC_FIELD.
Episoft have developed a new Form Configuration for the Family and Obstetric History section to have multiple questions about a maternity patient's family history. Setting the following form configuration (HISTORY_OBSTETRIC_FAMILY_HISTORY_SEPARATE_QUESTIONS) to True displays a number of additional questions. They are nested under the heading: 'Family History of:' and display with Yes and No radio buttons. They are: 'Heart disease *' 'Mental illness *' 'Diabetes *', 'Physical abnormalities *' 'Chromosomal abnormalities *', 'Other family history *' - this final field displays an additional 'Please specify *' field when Yes is selected.
A new Form configuration has been added which can hide the question regarding the patient having been previously induced. This is True by default for most epi-me customers but setting HISTORY_OBSTETRIC_PAST_HISTORY_INCLUDE_PREVIOUSLY_INDUCED to False will hide the question. This was previously visible by default.
A new Form configuration has been added which can hide the question regarding the patient having previous complication due to pregnancy. This is True by default for most epi-me customers but setting HISTORY_OBSTETRIC_PAST_HISTORY_INCLUDE_PREVIOUS_COMPLICATION to False will hide the question. This was previously visible by default.
A new Form configuration has been added which can hide the question regarding the patient having attended a special care nursery. This is True by default for most epi-me customers but setting HISTORY_OBSTETRIC_PAST_HISTORY_INCLUDE_PREVIOUS_SPECIAL_CARE_NURSERY to False will hide the question. This was previously visible by default.
A new Form configuration has been added which can hide the question of 'Do you have a birth plan?'. This is True by default for most epi-me customers but setting HISTORY_OBSTETRIC_PAST_HISTORY_INCLUDE_BIRTH_PLAN to False will hide the question. This was previously visible by default.
A new Form configuration has been added which can hide the question of 'Have you had any problems during pregnancy?'. This is True by default for most epi-me customers but setting HISTORY_OBSTETRIC_PAST_HISTORY_INCLUDE_PROBLEMS_THIS_PREGNANCY to False will hide the question. This was previously visible by default.
A new Form configuration has been added which can hide the question regarding details of a support person during pregancy. This is True by default for most epi-me customers but setting HISTORY_OBSTETRIC_PAST_HISTORY_INCLUDE_OTHER_SUPPORT_PERSON to False will hide the question. This was previously visible by default.
A new Form configuration has been added which displays the question: 'Do you have a history of pelvic trauma? *' This is False by default but can be added by setting HISTORY_OBSTETRIC_PAST_HISTORY_INCLUDE_PELVIC_TRAUMA to True.
Episoft have developed two new Form Configurations for the Previous pregnancies grid. The first allows the dropdown question on how past babies have been fed (breast or bottle) to be hidden. This is defaulted to True for all customers but setting it to False will hide the question. The Form Configuration is labelled: HISTORY_OBSTETRIC_PAST_PREGNANCIES_BABY_FEEDING.
The second form configuration is labelled HISTORY_OBSTETRIC_PAST_PREGNANCIES_COMPLICATIONS_AFTER_FREE_TEXT which includes a free text question on complications after birth.

Insurance Details page - New hint text on the Medicare Expiry field (EM-6458)
There is a new Form configuration to add hint text next to the meciare expiry fields to display the format of the expiry field to patients on the Insurance page. These will display when the following configuration is set to True: INSURANCE_ALTERNATE_MEDICARE_EXPIRY_LABEL
Insurance Details page - New Alternate 'Other Cards' section (EM-5919)
A new Form Configuration has been added which reconfigures the Other Cards section and displays each by default, INSURANCE_OTHER_CARD_ALTERNATE_DISPLAY_THREE. Setting this to True splits out the various 'Other Card' types into distinct questions as follows:
- Do you have a pensioner card? (Y/N) with subquestions: Valid pension card number and pensioner card expiry (it also displays a sample card as an image)
- Do you have a health care card? (Y/N) with subquestions: Valid health care card number and health care card expiry
- Do you have a Commonwealth seniors card? (Y/N) with subquestions: Commonwealth seniors card number and Commonwealth seniors card expiry
Insurance Details page - New field of Insurer's fax number added to WC, Third Party and Public Liability (EM-5919)
A new Form Configuration has been added at the request of one of our customers to include an 'Insurer's fax number' field for users who select Workers Compensation, Third Party and Public Libaility. This can be configured by setting INSURANCE_INCLUDE_INSURER_FAX_NUMBER to True.
Insurance Details page - PRFA address fields (EM-6011)
A new Form Configuration has been added at the request of one of our customers labelled: INSURANCE_INCLUDE_PAYMENT_ACCOUNT_POSTAL_ADDRESS
When set to True (it is False by default) this will display a mandatory question: ‘Postal address same as residential? *' (Y/N) located below the email address field (see screenshot below) - Note: This is changed from V2 as the address fields do not apppear unless ‘Relationship to patient *’ has been selected as ‘Other’ in V3

Legal Minor page - New Form Configurations to hide Custody Orders and include questions on Visitor and Phone Restrictions (EM-5950)
We have added three new Form Configurations to the Legal minor page for Paediatric admissions. The first (LEGAL_MINOR_INCLUDE_CUSTODY_ORDERS) is defaulted to True for epi-me customers but for customers who want to hide the question 'Are there any custody orders regarding the patient?' can set this to False. The second Form Configuration (LEGAL_MINOR_INCLUDE_SPECIFY_FOR_VISITOR_RESTRICTIONS) adds the following question when set to True: Are there any visitation restrictions we should be made aware of? *. The third Form Configuration (LEGAL_MINOR_INCLUDE_PHONE_RESTRICTIONS) adds the following question when set to True: Do you wish to have any restrictions on telephone? *
Admission Form Submission Page - New Form Configuration to make Patient rights and responsibilities consent mandatory (EM-6150)
Episoft have added new Form Configuration which allows our epi-me customers to make the patient's rights and responsbilities consent mandatory. This is governed by ADMISSION_SUBMIT_RIGHTSRESPONSIBILITIES_CHECKBOX_MANDATORY which when set to True makes the consent mandatory.
Health History - Relabel of Skin Integrity question wording to use correct clinical terminology of 'Injuries' (EM-5598 and EM-6465, Helpdesk #23241)
At the recommendation of one of our epi-me customers Episoft have universally relabelled the skin integrity question for those hospital customers who have it configured to 'Do you have or have you ever had skin injuries or pressure injuries? *'. This was done to accord with established clinical wording.
Health History - New Form Configuration to relabel Cancer question (EM-5785)
Episoft have added new Form Configuration which allows our epi-me customers to relabel the primary Cancer question to 'Cancer / lymphoma / leukaemia *'. This is False by default for all customers but can be configured by setting the following Form Configuration to True: HISTORY_SIGNIFICANT_CANCER_ALTERNATE_LABEL
This will then display the question to Patients as follows:
In addition to this a new form configuration has been added to make the 'Date of Inital diagnosis' on the Cancer section mandatory. This field only appears when the existing Form Config labelled: HISTORY_SIGNIFICANT_INCLUDE_CANCER_DATE_LAST_TX is set to True.
The new configuration is labelled HISTORY_SIGNIFICANT_CANCER_DATE_LAST_TX_MAKE_MANDATORY and is nested under the other configuration that sets the date as mandatory.
Health History - New Dynamic Text Item above Surgeries grid (EM-5785)
Episoft have added a new Dynamic Text item (number 92) above the Surgeries grid on the 'Other Significant History' page. Customers are able to enter their desired wording to display to patients making it clearer that they need to select the 'Add Previous Surgery' button. This wording is configurable using the Dynamic Text page of the Admin Portal.
Health History (Paediatric) - New Form Configurations on the Paediatric Infection Screening page (EM-5954)
A new Form configuration (HISTORY_INFECTION_INCLUDE_HOSPITAL_OS_LAST_12_MO) has been added to hide the 'Has the child been overseas in the last 12 months?' question. When set to True this question will display.
A new Form configuration has been created to relabel the respiratory question on the Paediatric infection screening page to 'Has your child recently had a cough, cold or contact with an infectious disease.' This is labelled HISTORY_INFECTION_COUGH_FEVER_RELABEL_INCLUDE_INFECTIOUS_DISEASE and is defaulted to False for all epi-me customers.
A new Form configuration has been created (HISTORY_INFECTION__CJD_RELABEL_FAMILY_HISTORY) this will display the alternate wording of 'Does your child, or any relatives, have Creutzfeldt-Jakob Disease (CJD)? *' when set to true, but is defaulted to False for all epi-me customers.
A new Form configuration has been created (HISTORY_INFECTION_EXTENDED_CJD_SECTION) it is defaulted to False. This displays the following questions: Does your child have an unexplained progressive neurological illness of less than 12 months duration? * (with a please specify field) and Has your child been involved in a ‘look-back’ for Creutzfeldt Jakob Disease (CJD) or have you been shown a ‘medical in confidence’ letter regarding their risk for Creutzfeldt Jakob Disease (CJD)? *
A new Form configuration has been created (HISTORY_INFECTION_INCLUDE_SPECIFY_FOR_IMMUNISATIONS) which when set to False will remove the 'Please specify' textbox that appears when the user hits no to 'Is your child fully immunised? *'. This is true by default.
Health History - New form Configuration to Hide Multi-Resistant Organism section on the Adult Infection Screening page (EM-5788)
At the request of an epi-me customer we have added a Form Configuration to hide the MRO section and all sub questions. This is by default False for all customers but can be set to True and is governed by the Form Configuration: HISTORY_INFECTION_INCLUDE_MRO
Health History - New form Configuration to Combine the Creutzfeldt Jakob Disease and Neurosurgery questions on the Infection Screening page (EM-5788)
At the request of an epi-me customer we have added two new Form Configurations to combine the previous neurosurgery and previous dura mater graft questions into a single combined question and hide the year field in which a patient was given human pituitary growth hormone. The first Form configuration is labelled: HISTORY_INFECTION_COMBINE_CJD_HORMONE_AND_NEUROSURGERY_QUESTIONS
and the following Form Configuration hides the 'Year' field for HISTORY_INFECTION_INCLUDE_CJD_GROWTH_HORMONE_YEAR
Health History - New form Configuration to amend the Recent Cold / Flu / Sore Throat question on the Infection Screening page (EM-5788)
At the request of an epi-me customer we have added a Form Configuration to amend the Recent cold / flu / sore throat question to more cutdown wording which reads: 'Recent cold *'. This question is amended by setting the following Form Configuration to True, and is False by Default for customers: HISTORY_INFECTION_FLU_ALTERNATE_LABEL
Health History - New form Configuration to hide the Vomiting and Diarrhoea questions on the Infection Screening page (EM-5788)
At the request of an epi-me customer we have added a new Form Configuration to hide the vomiting and diarrhoea question on the infection screening page, this will be True by default for customers and is labelled: HISTORY_INFECTION_INCLUDE_VOMITING_AND_DIARRHOEA
Health History - New form Configuration to add Herpes and Rubella questions to the Infection Screening page (EM-5899)
At the request of an epi-me customer we have added two new Form Configurations to includes questions on Herpes and Rubella/German measles to the infection screening page. The first Form Configuration is labelled HISTORY_INFECTION_INCLUDE_HERPES_SECTION_TYPES1AND2 and when set to True displays the following questions with associated Y/N radio buttons: 'Herpes Type 1*' and 'Herpes Type 2*'. This new question will trigger the Infection Alert.
The second Form Configuration is labelled HISTORY_INFECTION_INCLUDE_RUBELLA_SECTION_TWO_QUESTIONS_HAD_DISEASE_AND_BEEN_VACCINATED and when set to True displays two questions on the infection screening page: Have you had rubella (German measles) *' with radio buttons for 'Immune', 'Not Immune' and 'Not sure' and in addition the question: 'Have you been vaccinated against rubella (German measles)? *' with radio buttons for 'No', 'Yes' and 'Not sure'. This new question will trigger the Infection Alert.This configuration is False by default.
Health History - New form Configuration for Maternity Patients on the Lifestyle and Diet page (EM-6220)
On the Lifestyle and Diet page for Maternity admissions, we have added a new Form Configuration (HISTORY_LIFESTYLE_PREGNANT_INCLUDE_NUMBER_OF_WEEKS_ with a default setting of False. When set to True it will ask patients who select 'Yes' to are you or could you be pregnant a seaparate question albelled 'number of weeks'.
Health History - New form Configurations on the Lifestyle and Diet page (EM-5793)
At the request of an epi-me customer we have added a number of new Form Configurations to the Lifestyle and Diet page, these make changes to the existing questions and are available to epi-me customers after their next release.
The first is a change to question to the question on recreational drugs and relabels the question to 'Past history of drug dependency *' as well as relabelling the sub question. This is governed by the new form configuration: HISTORY_LIFESTYLE_USE_RECREATIONAL_DRUGS_ALTERNATE_LABEL which is False by default.
The second change is to the existing exercise frequency question which all customers currently have but can now be hidden as an option by using the Form Configuration: HISTORY_LIFESTYLE_INCLUDE_EXERCISE_FREQUENCY. This True by default.
The following optional Form Configurations make changes to the Aids, Impairments and Mobility section. The first adds two checkboxes to the 'Visual Aids / Impairments' question, 'Left' and 'RIght' so users can specify which eye has an issue or impairment. This is governed by HISTORY_LIFESTYLE_VISUAL_AIDS_INCLUDE_LEFT_RIGHT_CHECKBOXES which is False by default but available to epi-me customers optionally.
The second optional change allows for the 'Glaucoma' and 'Eye Lens' checkboxes under the same Visual Aids / Impairments question be hidden by setting the Form Configuration HISTORY_LIFESTYLE_VISUAL_AIDS_INCLUDE_GLAUCOMA_AND_EYE_LENS_CHECKBOXES" to False. This is true by default for customers.
The third optional change is the optional removal of the primary mobility question. This is governed by the new form configuration HISTORY_LIFESTYLE_INCLUDE_IMPAIRMENT_MOBILITY" and is by default True for epi-me customers.
The fourth optional change is a change to the 'Walking Aids' question which when set to True amends the question from a series of optional checkboxes e.g Walking Frame, Stick etc. to a (Yes/No) radio button with a mandatory 'Please specify' field. This is governed by setting the following Form Configuration to True: HISTORY_LIFESTYLE_AIDS_MOBILITY_WALKING_ALTERNATE_LAYOUT.
On the Lifestyle and Diet page there is a new optional Form Configuration which hides the Skin Conditions section and all the checkboxes within it e.g Eczema, Psoriasis etc. This is governed by the following form configuration and is True by default for epi-me customers. HISTORY_LIFESTYLE_INCLUDE_SKIN_CONDITIONS.
On the Lifestyle and Diet page there is a new optional Form Configuration which amends the dental section and asks about Dentures only, with (Y/N) radio buttons for 'Dentures - upper' and Dentures - lower *' and checkboxes for 'Full' and 'partial' nested under each. This can be enabled by setting the following Form Configuration to True: HISTORY_LIFESTYLE_DENTAL_SECTION_INCLUDE_DENTURES_ONLY_WITH_DENTURES_DETAIL
Lifestyle & Diet (Adult & Paediatric) - new hint text to add food allergies to the Allergies Grid rather than special diets (EM-5793)
This is only applicable to any customers with the standard Paediatric Lifestyle & Diet page, or the Adult Lifestyle & Diet page with HISTORY_LIFESTYLE_INCLUDE_SPECIAL_DIETS set to TRUE (i.e Special Diets being collected for Adults).
We have introduced helpful hint text to instruct Patient Portal users to enter any special dietary needs relating to food allergies on the Allergies Grid rather than as a special diet.
This is not subject to a configuration and has been added as a global enhancement.
Health History - New form Configurations on the Discharge Planning page (EM-5794)
There have been a number of new Form Configurations added to the Discharge Planning page that are available as optional changes to epi-me customers. They include a new Form Configuration that allows a hospital customer to hide the question: 'Do you have any caring responsibility for others?'. This is governed by the following Form Configuration which is set to True by default HISTORY_DISCHARGE_CARE_FOR_OTHERS_INCLUDE_SPECIFY
The Form Configuration (HISTORY_DISCHARGE_ADLS_RELABEL_COMMUNITY_SERVICES) when set to True relabels the 'Do you require assistance with daily activities?' question to: 'Do you receive community services? *'. and in addition hides the checkboxes in that section that are not community services (see below screenshot). This will be False by default for most epi-me customers.
The new Form Configuration (HISTORY_DISCHARGE_CONCERNS_HOME_RELABEL_PROBLEMS_CARING) when set to True relabels the caring responsibilities question as 'Do you have problems caring for yourself at home? *'. This will be False by default for most epi-me customers. In addition a new Form Configuration has been added that allows the associated 'Please specify' question to be removed when the following form configuration is set to False: HISTORY_DISCHARGE_CONCERNS_HOME_INCLUDE_SPECIFY.
There is a new Form Configuration which allows epi-me users to detail their relationship with the person taking them home by selecting from a drop down list (this displays for Day Stay patients only). This can be configured by setting the following Form Configuration to True: HISTORY_DISCHARGE_INCLUDE_RELATIONSHIP_OF_PERSON_TAKING_HOME. In addition epi-me hospital customers can collect a mobile phone number of the person responsible for collection by setting the following Form Configuration to True: HISTORY_DISCHARGE_INCLUDE_MOBILE_PHONE_DAY_ONLY.
At the request of one of our customers Episoft has also created a form configuration to hide the final 'any other medical conditions' question on the discharge planning page, this can be removed by setting the following to False: HISTORY_DISCHARGE_INCLUDE_MEDICAL_CONDITIONS. This is True by default for epi-me customers.
Health History - New form Configurations on the Obstetric History page (EM-5900 and EM-6236)
A new set of Form configurations have been added to the Obstetric History page and can be optionally configured by epi-me customers. The frist is a form configureation that sets the patient's partners name to a single field (as opposed to First and Surname as distinct fields) This can be configured by setting the following Form Configuration to True: HISTORY_OBSTETRIC_INCLUDE_PARTNER_NAME_ONE_FIELD_ONLY.
A new form configuration which displays a free text field labelled 'Other Relevant Past medical history' can be added to the Obstetric History page by setting the following configuration to True: HISTORY_OBSTETRIC_INCLUDE_RELEVANT_PAST_HISTORY_FREE_TEXT
A new form configuration to include the following question has been added: 'Are you having a planned caesarean section? *'. This can be added by setting HISTORY_OBSTETRIC_INCLUDE_CURRENT_PREGNANCY_IS_A_CAESAREAN to True.
At the request of one of our customers we have added a new form configuration to allow for the question 'How many weeks pregnant were you at the time of your first appointment' to be hidde. This can be implemented by setting the following Form Configuration to False HISTORY_OBSTETRIC_INCLUDE_FIRST_VISIT_TO_DR_WEEKS_PREGNANT. This configuration is True by default for epi-me customers.
Episoft have developed a new Form Configuration for the multiple pregnancy question to allow users to enter the number of expected babies, with the wording 'If you are expecting more than one baby, how many?'. This can be configured by setting the following Form Configuration to True: HISTORY_OBSTETRIC_ALTERNATE_QUESTION_FOR_MULTIPLE_BIRTHS_AS_NUMERIC_FIELD.
Episoft have developed a new Form Configuration for the Family and Obstetric History section to have multiple questions about a maternity patient's family history. Setting the following form configuration (HISTORY_OBSTETRIC_FAMILY_HISTORY_SEPARATE_QUESTIONS) to True displays a number of additional questions. They are nested under the heading: 'Family History of:' and display with Yes and No radio buttons. They are: 'Heart disease *' 'Mental illness *' 'Diabetes *', 'Physical abnormalities *' 'Chromosomal abnormalities *', 'Other family history *' - this final field displays an additional 'Please specify *' field when Yes is selected.
A new Form configuration has been added which can hide the question regarding the patient having been previously induced. This is True by default for most epi-me customers but setting HISTORY_OBSTETRIC_PAST_HISTORY_INCLUDE_PREVIOUSLY_INDUCED to False will hide the question. This was previously visible by default.
A new Form configuration has been added which can hide the question regarding the patient having previous complication due to pregnancy. This is True by default for most epi-me customers but setting HISTORY_OBSTETRIC_PAST_HISTORY_INCLUDE_PREVIOUS_COMPLICATION to False will hide the question. This was previously visible by default.
A new Form configuration has been added which can hide the question regarding the patient having attended a special care nursery. This is True by default for most epi-me customers but setting HISTORY_OBSTETRIC_PAST_HISTORY_INCLUDE_PREVIOUS_SPECIAL_CARE_NURSERY to False will hide the question. This was previously visible by default.

Episoft have developed two new Form Configurations for the Previous pregnancies grid. The first allows the dropdown question on how past babies have been fed (breast or bottle) to be hidden. This is defaulted to True for all customers but setting it to False will hide the question. The Form Configuration is labelled: HISTORY_OBSTETRIC_PAST_PREGNANCIES_BABY_FEEDING.
The second form configuration is labelled HISTORY_OBSTETRIC_PAST_PREGNANCIES_COMPLICATIONS_AFTER_FREE_TEXT which includes a free text question on complications after birth.
