Blood clots | If patient answers yes to having had previous blood clots or on current treatment for cancer. | Displays location of blood clot, and any provided details |
BMI < 18 | BMI as calculated and displayed on the Lifestyle & Diet page | Displays the exact BMI |
BMI > 35 | BMI as calculated and displayed on the Lifestyle & Diet page | Displays the exact BMI |

Cancer | Patient has or previously had cancer | Displays cancer diagnosis date, location of cancer and whether treatment is current or complete. |
Cardiac | Yes to any current or historical patient heart condition (excludes family history of cardiac disease), any history of cardiac surgery and blood clots. Excludes blood pressure and cholesterol issues. | Displays what question/s triggered the alert |
Chronic pain | Patient has chronic pain. | Displays patient specified detail about chronic pain |
Cognitive risk (>65 years or memory loss) | If patient answers yes to memory loss/confusion/dementia OR is > 65 years measured at Admission Date | Displays questions which triggered the alert. |
COVID | Yes to recent COVID diagnosis (within 30 days of admission only if reported within main eAdmission). Also, Yes to any clinical indicators of infection on the Current Infection Screening follow up form. | Includes COVID diagnosis date, any COVID-like symptoms or whether they are a close contact in past 7 days |
Diabetic | Yes to Diabetes, regardless of diabetes type | Displays diabetes type, how it is managed and how it is monitored. |
Discuss needs before admission | If patient answers yes to this question on the discharge planning page | Displays preferred contact number |
Falls risk | If patient answers yes to recent falls or impairment of mobility | Displays whether fallen in last 12 months, and how many times. |
Has Allergies | Any allergy entered in the Allergies & Details page. | Displays 'Patient has listed allergies' if they have allergies entered into the grid / lookup as well as specific prompted allergy questions (Latex, Adhesive tape, Medication, , Food, Adrenaline autoinjector and Allergy action plan). If Patient User reports 'Patient has allergies' but then doesn't report any of the above, text will be 'Patient has allergies (details not recorded)' |
High Risk Medications | Yes to Diabetes or Weight-loss medications on the Allergy & Medications page | Displays what medications the patient has reported, if their Admitting Dr is aware and if they have been instructed to cease (and if so, what is the cessation date). |
Hospital/ACF admissions o/s or local last 12 (or 3) MO | Yes to admitted to hospital outside WA in last 12 months, or admitted to overseas hospital in last 12 months | Displays detail of overseas or interstate admission including location, reason and what type of facility |
Infection risk | Yes to any questions on the Infection Screening page (bar overseas and/or interstate hospital/ACF admissions and COVID questions) or No to Immunisations up to date | Displays questions which triggered the alert eg. CJD, MRO, travel to infectious areas. |
Latex allergy | If patient answers yes to specific latex allergy question and/or add latex allergy to their allergy list | Displays reaction to latex (if applicable). |

Skin Integrity | Yes to any skin problems, leg ulcers or pressure ulcers, mobility issues as a result of respiratory or bone & joint issues, lost weight without trying, kidney disease or renal failure, bladder problems. | Displays skin problems and description of skin ulcer, bladder or kidney issues, as well as describes any impairment of general mobility or impairment of mobility caused by respiratory issues. |
