General, Maternity and Paediatric Health History PDFs - missing data, erroneous data and misleading data labels (Helpdesk #35993, EM-7838)

#	Problem	Fix
1	General "Cardiac problems eg heart attack, chest pain/angina, congestive cardiac failure, rheumatic fever" missing on PDF	It’s included with label “Cardiac problems”
2	Maternity "Do you have or have you ever had any cardiac abnormalities or conditions?" missing on PDF	It’s included with label “Any cardiac abnormalities or conditions”
3	Maternity Pacemaker "Make" and "Model" appear on PDF but aren’t questions on the eAdmission	Remove the PDF labels
4	Maternity "Artificial or organic heart valve replacement" - "Brand", "Date last checked" and "Cardiologist that performed the check" subquestions are missing on PDF	They’re included with the following labels: Brand: Date last checked: Checked by:
5	Maternity "Other Heart Surgery" appears on PDF but isn’t a question on the eAdmission	Remove the PDF label
6	Maternity "Other implants or devices" missing on PDF	It’s included with label “Other implants or devices”
7	Maternity Other implants or devices ‘Please specify’ is incorrectly displaying on same line as Grafts	Reposition it to same line as Other implants or devices (see immediately above)
8	General "Do you currently have or have you ever had any other respiratory problems e.g. shortness of breath / bronchiectasis / asbestosis ?" prints to a misleading label ‘Shortness of breath’	Label should be “Other respiratory problems”
9	Maternity "Back / pelvic / neck / hip disorders" missing on PDF	Include it with label “Back / pelvic / neck / hip disorders”
10	Maternity Diabetes sub questions missing from PDF a) "If known, what gestation were you at the time of developing gestational diabetes i.e 20 weeks pregnant?", and; b) "Other treatment" checkbox c) "Do you have any side effects related to your diabetes (e.g. reduced sensation in feet)?"	Include them with the following labels: a) "Gestation at time of developing diabetes" b) New value printing against "Managed by" label c) Diabetes side effects
11	General and Maternity Missing the question and response of "Assistance with medication question" on the PDF	Include them with label “Assistance with medications”
12	Paediatric Bladder problems sub questions missing from PDF a) "Incontinence" checkbox b) "Any other bladder problems" and it's response	Include them with the following labels: a) "Incontinence" printing against "Bladder problems" label b) "Any other bladder problems?"
13	General "Other Heart Surgery" is a misleading label for the Cardiac surgery question.	Label should be “Cardiac surgery”
14	Paediatric Diabetic Side effects is shown on the PDF but isn’t a question on the eAdmission	Remove it.
15	Maternity 'Cardiac surgery eg: pacemaker, implants or devices, prosthetic heart valve, grafts, stents’ missing on the PDF	Included with label ‘Cardiac surgery’
