NEW FEATURES:
PATIENT PORTAL
Amendment of patient Sex field to 'Patient Sex' recorded at birth - Burnside ONLY (COVERED IN PREVIOUS EPI-ME RELEASE NOTES) (EM-6978)
A New Form configuration (PERSONAL_DETAILS_SEX_RELABEL_ has been added which when set to True relabels the Patient Sex field on the Personal details page to: ‘Patient Sex (recorded at birth) *’.
Additional tickboxes in Cancer Treatment section of forms - Burnside ONLY(COVERED IN PREVIOUS EPI-ME RELEASE NOTES) (EM-7338)
A universal enhancement for epi-me customers has been applied to the cancer section of the Health History forms to incllude cancer treatments not covered by the existing questions. Two new tickboxes under the ‘Please specify Cancer treatment *’ section have been added, labelled:
Universal Enhancement to Primary Mental Health question for epi-me customer and New Form Configuration for additional details on mental health treatment (EM-6980)
At the request of an epi-me customer and after consultation with all customers, the primary Mental Health question on the second page of the health history form in the 'neurological and mental health sectionhas been universally changed to more inclusive wording. It previously read:
In addition a new optional Form Configuration labelled: (HISTORY_NEURO_MENTAL_HEALTH_MAKE_CLINICIAN_MANDATORY_AND_RELABEL_WITH_DATE__ADDITONAL_FIELD) has been added which when set to True does the following:
See screenshot below with the new and relabelled fields highlighted. Note: In order for this Form configuration to function as expected, the existing Form Configuration (HISTORY_NEURO_INCLUDE_ADDITIONAL_MENTAL_HEALTH_QS must be set to True.
'Do you use an eCigarette or Vape?' question can be added using a new Form Confgiruation to display (EM-6982)
EpiSoft have developed new form configuration labelled: HISTORY_LIFESTYLE_DIET_INCLUDE_ECIGARETTE_VAPE adds a new mandatory question to the Lifestyle and Diet page directly under the smoking question relating to vapes and eCigarettes.
Expanded questions under Chronic Pain section on the Lifestyle and Diet page (EM-6982)
For hospital customers with the chronic pain question enabled on their forms, a new form configuration HISTORY_LIFESTYLE_INCLUDE_CHRONIC_PAIN_INCLUDE_ADDITIONAL_QUESTIONS can be configured to display additional questions regarding pain physician and pain clinic on the Lifestyle and Diet page in the ‘Other General health details’ section for Chronic pain. Note: HISTORY_LIFESTYLE_INCLUDE_CHRONIC_PAIN must be enable to display these questions.
When configured this change adds the following new questions under 'Do you have chronic pain? *':
New Palliative Care question in the 'Other General Health details' section of the Lifestyle and Diet page (EM-6982)
At the request of one of our customers, EpiSoft has added a new form configuration (labelled HISTORY_LIFESTYLE_INCLUDE_PALLIATIVE_CARE). When enabled this adds a new standalone non-mandatory question to the Lifestyle and Diet page under the ‘Other General health details’ section concerning Palliative Care.
Have you been referred to palliative care? (Yes/No) - If yes, is selected then display the following questions:
New Dynamic text fields above the 'Past pregnancies' grid on Obstetric History page (EM-6986)
In order to give epi-me customers greater autonomy over sections of instructional text for patients throughout the forms Episoft have added a new Dynamic text field (ID 96) to allow hospital customers to edit and update the instructions above the past pregnancies grid on the Obstetric History page. This page only displays for hospitals with the Maternity forms configured and only displays for Maternity patients.
Amendment in Assistance with Daily Activities section of the Discharge planning page to 'Further details / other assistance’ question (EM-7128)
EpiSoft have made a universal change to relabel the free text field under the ‘Do you require assistance with daily activities?’ section (which should display if ‘Yes' is selected to this parent question) to include service provider details if they are applicable to the patient. The field has been relabelled from:
Amendment to follow-up question for Falls risk on the Lifestyle and Diet page (EM-7164)
A new Form Configuration ( HISTORY_INCLUDE_FALLS_QUESTION_SPECIFY_RELABEL) has been added at the request of one of our customers to amend the please specify wording of the falls question on the lifestyle and Diet page. When configured this relabels the current wording from:
New Blood transfusion consent question for patients on page 2 of the Health History Form (EM-7337)
At the request of an epi-me customer, and to account for patients with cultural, personal or religious issues with blood transfusions, EpiSoft have added a new form configuration (HISTORY_BLOOD_TRANSFUSIONS_CONSENT) which when true displays the following mandatory question on the second page of General Health History (cont). - Blood Disorders' section of Adult Health History pg 2) at the bottom of the section but above the specialist name:
New Form Configuration to add additional 'Details' field in the Cancer section (EM-7339)
A new form configuration has been added to the forms at the request of an epi-me customer (HISTORY_SIGNIFICANT_INCLUDE_CANCER_DETAILS_FIELD) which when configured adds an additional 'Details' field to the cancer section, allowing patients to enter further information which mightn't be covered in the other questions.
New Form Configuration to Brace and/or wires question to the Dental section (EM-7339)
For epi-me customers using the extended Dental section on the Lifestyle and Diet page (HISTORY_INCLUDE_DENTAL_SECTION), the hospital can now optionally includes a question concerning braces and/or wires in the same format. HISTORY_INCLUDE_DENTAL_BRACES_WIRES being configured will display this additional Yes/No question to patient users on the General Admission (Adult) form.
Amendment to Cancer section to place name of specialist(s) at the bottom of the section (EM-7678)
To align the Cancer question and section display order with all other sections throughout the forms, EpiSoft have relocated the 'name of specialist(s)' field to appear under the cancer treatment tickboxes on the forms and on the PDF. This is to ensure the form has logical flow with the name of specialist question being asked in the treatment section. Note: This requires HISTORY_SIGNIFICANT_INCLUDE_CANCER_SPECIALIST_NAME to be configured on the hospital customer portal.

Obstetric History PDF - Issue with Past issues with Pregnancy Section (EM-8031)
The following questions and responses were missing on the Obstetric History section of the PDF for Maternity patients. This has now been fixed. Please note, not all hospital customers have all of these questions configured on their respective pages.
Lifestyle and diet page - When response to question "Do you have chronic pain? is changed from Yes to No, the sub questions do not hide until the page is saved - (EM-8037)
EpiSoft have fixed a display bug on the Lifestyle and diet page which arose when a response to the question "Do you have chronic pain? is changed from Yes to No, the sub questions did not hide until the page is saved. This has been fixed and will no longer be an issue for patients.
Preadmission Blood tests question responses not updating on forms and PDF after subsequent save (EM-8038)
EpiSoft have applied a fix to the History Forms and PDF for General admissions which erroneously saved previous entries of the patients preadmission Blood tests. If the patient had indicated they had had previous blood tests then after saving navigated back to select No on the same question. The page was not clearing their response from the forms. This has now been fixed.
Other Significant History page - Lymphoedema and Have you seen a Physician responses were missing from the PDF for some hospital customers (EM-8042)
EpiSoft have applied a fix that meant hospital customers with the Lymphoedema and Have you seen a Physician questions configured on the forms, were missing from the PDF.

Universal Enhancement to Primary Mental Health question for epi-me customer and New Form Configuration for additional details on mental health treatment (EM-6980)
At the request of an epi-me customer and after consultation with all customers, the primary Mental Health question on the second page of the health history form in the 'neurological and mental health sectionhas been universally changed to more inclusive wording. It previously read:
- ‘Do you have any mental health conditions such as anxiety, depression or psychosis, Post-natal depression or other? *’
- 'Do you have any mental health conditions such as anxiety, depression, PTSD, psychosis, bipolar, ADHD, post-natal depression or another mental health condition? *'
In addition a new optional Form Configuration labelled: (HISTORY_NEURO_MENTAL_HEALTH_MAKE_CLINICIAN_MANDATORY_AND_RELABEL_WITH_DATE__ADDITONAL_FIELD) has been added which when set to True does the following:
- Relabels the existing field 'specialist name' to 'Name of psychologist or psychiatrist',
- Makes the 'Name of psychologist or psychiatrist' question mandatory
- Adds a new ‘date of last review’ field.
