Changes to the HONOS form


Subscale scores has been added, the subcales algorithm is Behaviour = sum of first 3 questions , impairment= sum of questions 4-5, symptom= sum of questions 6-8 and social= sum of 9-12



New Life Skills Profile (LS16) Form

The new form contains 16 questions that are answered by moving a slider scale. You must answer all 16 questions to save the form as an incomplete score cannot be captured.
The score is displayed down the bottom of the form. There is a subscale score for each of the 4 categories.



Risk Assessment Mental Health Form Changes

The following changes have been made to the 'Risk Assessment Mental Health'

• All data (radio buttons to carry forward data.
• Remove the rating of risk drop down box for all questions when the Yes radio button is selected.
• Rating Risk drop down box to be replaced with a Comments text box with as many characters that the form allows when Yes is selected as an answer (if possible a word wrap scenario)
• Addition of a Radio button called Unknown for all questions that have radio buttons as answers
• Removal of the following Comments text boxes at the end of each section: Comments re risk to self, Comments re risk to others, other accommodation details & Comments re environmental risk.
